Hi, we use Employee Self-Service for our Annual and New Hire benefits enrollment. I am trying to figure out if possible how to configure ESS Benefits Enrollment to handle a Spousal Surcharge for our Medical plan. We also would like to add new form/screen to our ESS Benefits Enrollment process that would display Spousal Surcharge Affidavit verbiage and have the employee select either a Yes or No on the affidavit which would update the HR11 Ben Flags->Spouse Coverage Health Coverage field/flag. Is there anyone that has anyone done this type of setup for a medical plan Spousal Surcharge?
